you need to go to moca site. you go to one of the clinics listed they take your fingerprints and photo. you go see the doc twice (it can be in the same week) pay the fee and you get your card. if you have been diagnosed with one of the approved medical conditions listed bring that with you to show you were previously diagnosed and it will save you the cost of a exam.
You have to get your permit/licence from the state. You have to see a doc a couple times and get your fingerprints done, a passport photo and of course pay the fee
