If you contact Phoenix doctors they have a great program. They are only open for a few hours during the day andd close by 13:00. If you have EBT and a card with your name on it: The doctor's visit is only $75 and you still pay the state $150. I am sure you can't find this price many places. I suggest you go there and they have a chiropractor on site and several doctors to document your medical condition. Hope this helps.
